Access the care you need
Trans Care
We are here to support patients with gender dysphoria, through to transition and afterwards. We are able make a referral to a specialist gender clinic. We are not able to prescribe hormones until patients have been seen by a specialist. Afterwards we work in conjunction to prescribe and monitor your treatment (shared care). We appreciate that NHS waiting lists are long and we can help during this time, including mental health support.
We are not able to enter into shared care prescribing for patients using private gender services based outside the UK or not registered with the Care Quality Commission.
Carers
Are you a Carer? Please let us know, we want to be able to help
A carer is someone who provides unpaid care by looking after someone who is older, disabled or seriously ill in their day-to-day life. This could be a family member or a friend. It can be challenging to access care for yourself and /or the person you are caring for and we want to make it easier.
At Kingston Health Centre we offer
- An annual carer health check appointment
- Flu vaccination
- Information and advice about local support- you can book with our Social Prescribers or check our dedicated section
- Flexibility around appointments that work for you- we have a Care Coordinator who can help with this
- Keeping yourself healthy- see our Health and Wellbeing Coaches
Learning Disability
We offer a yearly health check with the GP (doctor). You can come on your own or with your carer. At the end of the appointment the doctor will give you a Health Action Plan. This will say what you, your carer and your doctors are going to do to help you stay healthy.
Have a look at the form below. It is a good idea to fill it in as best you can before your appointment. You can bring it with you or email it to us before your appointment.
- Pre Annual Health Check Questionnaire
- Easyhealth.org.uk is full of brilliant information that is easier to understand.
Veteran Friendly Practice
We are an Armed Forces veteran friendly accredited GP practice. If you are a Veteran please let us know.
This means we have a dedicated doctor who has a specialist knowledge of service related health conditions and Veteran specific health services. This is important in helping Veterans to get the best care and treatment.

Homelessness
We recognise that homeless patients find it difficult to register with a GP and access good healthcare. We work closely with local charities to ensure that patients can register easily and be seen the same day to address their health concerns promptly.
Our social prescribers can help with advice about housing.
